Executive Summary
Problem Statement:
Standard knee-high compression stocking orders averaged 6-8 weeks for the Veteran to obtain at a wide variation in pricing ($24.06-$90.77 per pair).
Business Case:
At the inception of this project, we had no established Veteran Hospital (VA) staff trained to fit the 320 standard knee high (8-15mmhg, 20-30mmhg and 30-40mmhg) out of 360 plus annual requests for Veterans to obtain a form of compression stockings. Compression stockings are issued for a variety of conditions to help improve venous blood flow and decrease edema in the extremities. Such conditions could include post-surgical, lymphedema, venous ulcers, venous disease and general edema for example. There was no established staffing, clinic, standard operating procedures (SOP) or standardization of any kind to support issuance of standard knee high compression stockings at the VA. As a result service was provided at a higher cost per pair with lengthy delays of up to 6-8 weeks with 46% of orders not being filled.
Performance Levels:
• Time from consults issued to closure of purchase order pre requested. (long waits)
• Not using of national contract items. (Cost)
• Not using VA clinical staff to fit Veterans to stockings.
Root Cause:
A lack of inventory to issue from combined with a lack of trained staff to measure and train Veterans to allow for in house issuance of standard knee high stockings was noted. As a result, this lead to a reliance on outside vendors to order stockings through. This required the stocking vendor in a timely manner to complete all the scheduling, measuring, issuing and then billing the VA.
Improvement Options:
• Identify clinical staff already available at VA whose scope of practice allows for the training and measuring of compression stockings.
• Develop training programs to competency VA clinical staff.
• Utilize VA National contract compression stockings as working inventory.
• Establish process to place consult and record measurements and trainings.
Control Plan:
• Train clinical supervisors to train new hires and re-competency staff.
• Created PAR levels for all stock items
• Created new process for recording information.
• Train all staff in new process

The initial problem was identified by Betty Bop, Dr. Michael Urban, Primary Care providers and the voice of the Veterans.

Goals
• Reduce wait times for Veterans to obtain standard knee stockings from 6-8 wks. to same day by 5/30/18.
• Reduce number of standard knee high stockings order requested not fulfilled by 25% by 5/30/18.
• Reduce price variations by $70 per pair for standard knee high stockings by 5/30/18.
In order to determine what the actual problem was, we identified our measures as;
• The cost per pair of stockings
• Days from consult placed to when closed.
• Number of consults not fulfilled.
We also identified our customers as:
• Clinicians who want timely issuance of standard knee high stockings.
• Veterans who want timely issuance of standard knee high stockings.
And our suppliers are:
